New Patient Registration

If you wish to register with the practice you should collect registration forms from the surgery.

Alternatively download the forms via the links below. Please ensure BOTH forms are completed before returning to the surgery.

The new patient questionnaire gives us an indication of your past medical history and allows us to provide appropriate care while we wait for your medical records to be transferred from your previous practice. Please either e-mail your registration forms to gram.gilbertroadadministrator@nhs.scot or hand them in at Reception.  It will take 2 days to register a new patient.

Change of Personal Details

When you are moving house, tell us your new address and telephone number immediately. We will then let you know if it's still in our practice area. If not, and you've moved out of our practice area, register with another doctor as soon as possible. Don't delay or there may be problems if you need a doctor urgently. After you re-register, your records will be transferred automatically to your new doctor.
