SICKNESS CERTIFICATES

No certificate is required under DSS regulations if you are off work for less than 4 days due to illness.

If you’re off work for up to 7 days, complete a self-certification form, available from your employer. If you’re off work for more than 7 days, you’ll need a certificate from your doctor - please contact the practice and you will be advised if you require an appointment.

If you’re claiming benefit from a private sickness or accident insurance policy, your doctor will complete the insurance company's claim certificate. A fee is charged for this service.
